Measurably Better Health
Monitoring community health status to identify and solve community health problems is a core public health function. Your Board of Health, in conjunction with Montgomery Township Health Department, works with regional partners to conduct a variety of assessments. We collect data from local sources as well as state and national datasets to help make evidence-based decisions to help our towns stay healthy.
The Community Health Assessment (CHA) evaluates what factors are affecting our neighborhoods’ health. What’s making our neighbors sick, or helping keep us all healthy? Mercer County CHA Link
The Community Health Improvement Plan (CHIP) identifies ways Health Departments, Hospitals, and community groups will work together to address what is impacting neighborhood health. Mercer County CHIP Link
